Plate Project Request Form

Plate Project Agreement

By borrowing plates or any other item from the Palouse-Clearwater Environmental Institute’s (PCEI) Plate Project I understand that as the borrower, I am responsible for any damage that may occur during the duration of the lease of these items. PCEI and Heart of the Arts, Inc. (HAI) are not responsible for broken items or any physical or bodily harm.

I agree to:

  • Pick up the items at the 1912 Center at 412 E 3rd Street, 9am – 6pm Monday-Friday the day prior to my event date.
  • Return items in the same bins provided and to the benches in the upper hallway of the 1912 Center (where they were picked up) within a few days following my event.
  • I will clean and sanitize all items before returning them.
  • If an item is lost or broken, I will replace it with a comparable item.

While we work to have dishes in ready-to-use condition, PCEI and HAI are not responsible for the cleanliness of the items borrowed from the Plate Project. You may wish to wash and sanitize all items before your event.

NOTE:  Please mail your Plate Project donation to PCEI at PO Box 8596, Moscow, ID 83843 (please do not leave checks at the 1912 Center).   You can also make a donation using our secure online payment form.  If you are not sure how much to donate, consider what you would regularly spend on renting and/or buying items for your event and then donate a percentage of that amount. Typical donations range from $10-$100. Donations are used to support the growth of the Plate Project.
